Meal Accommodations for Students
Students who need accommodations for their meals must turn in the Diet Prescription for Meals at School form to their respective school lunch manager. To be valid, the form must be completely filled out (except for the last portion of the form that is to be filled out by the lunch manager and the nutrition services director). Students whose disabilities or special dietary needs are life threatening must have the form signed by a physician. All other disabilities or special dietary needs may be signed by a physician or a recognized medical authority (physician assistant, registered nurse, registered dietitian). A new form will be required only if the student's condition changes over time.
