Medicaid Application Form

Medicaid is a federally funded program with a state match for people at or below the poverty level.  Eligibility is based on income and assets. Statistics indicate that students who do not receive regular medical care miss more school than students who do, often for simple ear infections and dental problems. 

Jordan School District Health Services Staff will publicize the open enrollment for Children's Health Insurance Program (CHIP), Primary Care Network (PCN), and Utah's Premium Partnership for Health Insurance, as well as the benefits of Medicaid. Health Services staff members will facilitate the application process enabling families to get the services they need more quickly. In addition, our department will educate district staff to allow them to identify and refer appropriate families to Health Services.
