Utah's Children's Health Insurance Program (CHIP)
 
What is CHIP?
  CHIP is a state health insurance plan for children. Many working Utah families who don’t have other health insurance qualify for CHIP.
   
 What does CHIP cover?
 
    •    Well-child exams
    •    Immunizations
    •    Doctor visits
    •    Hospital and emergency care
    •    Prescriptions
    •    Hearing and eye exams
    •    Mental health services
    •    Dental care
 How do I qualify?
  Children qualify for CHIP based on income and family size. For example, a family of four earning up to $38,700 per year may qualify (see FAQ's for more information).

In addition, children must be:
 
  •    Under age 19
  •    U.S. citizens or legal residents
  •    Not covered by other health insurance
 How much will CHIP cost?
  Depending on your income, you may pay up to $60 every three months. Monthly options are available. You may also pay small co-pays for services like a visit to the doctor.
   
 How do I apply?
  Apply for CHIP during open enrollment. To find out when open enrollment will be held, watch and listen for TV, radio, and other announcements. You may also call 1-877-KIDS-NOW (1-877-543-7669) or check the CHIP website.
   
 What happens after I apply?
  You may choose the CHIP health plan that is best for your children. Your current doctor may be a CHIP provider. Ask you provider if they accept CHIP and choose the plan that covers their office. For more information, please visit the MY CHIP page.
