Children's Health Insurance Program (CHIP)
Opens July 2, 2007, and will remain open until further notice. CHIP is a state health insurance plan for children. Many working Utah families who don't have other health insurance qualify for CHIP.

The Health Services Department is located in a portable office building behind Jordan Valley School in Midvale.  We are able to assist families in the application process for Medicaid and CHIP – call Azure Thompson at 412-2519 for more information.    We look forward to serving the families and children in Jordan School District.
